What Could Be Causing My Severe Lower Back And Buttock Pain With High ESR?

I have a lower back pain from last one month its like cutting pain, leading to buttocks, pain is severe not able to sit also , as suggested i under went blood test, it shows high ESR i.e. 81 . Kindly suggest . In my profile I have already attached the comprehensive blood report.
