You have asked a very important question related to fluid balance in CRF and CHF patients.
In renal patients, where GFR is at least 30% preserved, 1 to 1.5 lit. water may be allowed in excess of daily urine output.
But, water requirement calculation is very complex in advanced
CKD or CKD 5 patients. The water intake should be adjusted by a renal dietician based on type of food, presence of fluid overload etc.
Not only water, salt restriction (Weight gain is an important marker of volume overload in these patients.
