The picture that you present is common in
PCOS and is due to
hormonal imbalances. Prolonged exposure to
estrogen leads to overgrowth of the uterine lining and then as there is no
progesterone due to absent ovulation, the lining grows un inhibited upto the point where it cannot grow any longer. At this time it sheds off due to lack of blood supply and nutrients in a haphazard manner. This produces irregular or prolonged bleeding.
The spotting may be due to the regular periods that came and the lining is now low. Again there is estrogen excess and some part sheds, the other doesn't producing spotting.
If you are planning to conceive, you must visit an
infertility specialist along with your husband. An ovulation study will be needed along with tests to look for open tubes. If there is no ovulation drugs may be needed to induce ovulation. Laparoscopic drilling of the ovarian cysts also produces good results. All of these can be discussed with your doctor and the management is usually step by step.
