Im a male 23 ..I have a red bump on my right nipple that re-ocurrs every other week for the past 4 months.. I changed soaps about 5 times and it still happens, Its sore to touch .. I even bought new shirts and it still happens.. nothing triggers it. It just happens.. what is this???

There are two possibilities of the sore bumps as either simple folliculitis due to infection of the hair near the nipple area or it can be part of simple eczema over the nipple area.
Both of the conditions are totally benign and never turn out to be a serious issue any time in life. You can meet a dermatologist who can guide you for the best of the possibility.
Till that time you can apply a mixed cram containing fucidin with mometasone in it. Apply it in a very thin layer so that it should not be visible to the naked eye.
