Hi, I can understand your concern for round, raised itchy circle on top of the foot.
After reading your query I can make our that it might be simple lichenification of the area. It means a slight thickening of the skin which is quite common to find in front of toe area because of repeated friction and rubbing form the opposing surface of shoes and sleepers.
DO not try to pinch or pop the area other wise can get infected. You can apply a moisturizing cream over the same for 3-4 times. The main treatment is by a mild steroid cream containing Fluticasone or Mometasone in it. You can apply this cream at night time in a very thin layer. Do not apply beyond 14 days without dermatological guidance. Do not wear tight fitting or lose fitting shoes as can cause more prominence of the area.
Take care.
