What Could Cause A Painful Dent In The Shin With No History Of Injury?

Hi, my wife just noticed a random dent in her shin, it is painful to touch. She has had no injuries to this area. I’m wondering what some causes might be. Brief medical: Diabetic, has has a positive ANA with speckled pattern, waiting on lupus confirmation (which would answer a lot of other medical issues)
