Hello

Alv stands for apicolordotic view. There is suspicious densities in left apical area,so you are advised for apicolordotic view.It is important to rule out infective etiology.
Apicolordotic view is a special view of chest X-ray to visualize apical region of lungs.
This view is necessary because apical region is covered by bony ribs, clavicle etc.Normal chest X-ray (PA)can miss the findings,so you are advised for this view.
Proper treatment depend upon findings.

Other findings like both CP angles, bony thorax, diaphragm, heart size appears normal.

Take Care
