What Could Excess Menstrual Bleeding Suggest?

Hello , im a twenty year old female and have had constant period issues . Ranging from coming every two weeks...then never again for up to one year , recently ive been noticing skin in the blood as well changin colour (Orange). It can be extremely heavy to the point were i feel faint or very light almost not there .
