What Could Loose Bones In MRI Report Suggest?

I have sever ankle pain and ive had an mri that came back with a lot of results one a loose bone and a torn ligament (a minor one) and also some scar tissue. The problem I have is my doc thinks my pain is coming from my ankle instability. But it also comes and goes im just not sure I want surgery any suggestions.
