What Could Menses With Painful Cramps Suggest?

I am thirty years old and trying to get pregnant. I normally have a very regular period but it is heavy and crampy.This month it came at the correct time but was almost like spotting for the first 5 days and day 6 has been heavy and extremely painful (back, cramps, weakness). Could I be suffering a miscarriage? Should I take a pregnancy test?
