Dear sir/madam, history wise you have more than 3 risk factor I.e age, diabetes, hypertension, previous bypass surgery. Any chest pain in you should not be neglected. By your description the pain more looks like heart related. Its better to get evaluated by cardiologist. Get ECG, echo, tmt ( if needed ) and basic blood investigations ( RBS, UREA, CREATININE, HAEMOGLOBIN ETC ). Have a cardiologist opinion.
