What Could Painful Menstrual Cramps With Fatigue Suggest?

I've been having menstrual like cramps throughout the entire month for the past year or two. I've also been experiencing extreme fatigue & lately forgetfulness. My menstration is extremely painful & prevents me from going to work or class. What could this be?
