What Could Recurring Swelling In Testicles Indicate?

My son went to the er 2 days ago. He was not feeling well. His sugar must have been up but any how he was stating that his testicles was hurting and they were swelling up I had him to go back to the er but he would so he decided to just soak them in warm water and witch-hazel he said they went down but now they have swollen again.
