What Could Swollen Thumb With Bumps On Skin Suggest?

I have severe pain and swelling in my thumb. It started as a small red area that has gotten progressively worse. In the beginning it was just tender to the touch, but now my whole thumb is swollen, severe shooting pain in my thumb, warm to the touch, and a small white bump under the skin. What could this be? Should I go to the ER?
