What Do Dark Patches On Cheekbones Indicate?

Hello, I have large dark patches on my cheekbones and some small ones on other places of my face. Is there a health concern that I should be aware of? How can I safely lighten them? I am religious about protecting my skin with sunscreen, large sun glasses, avoiding sun and hats, yet I still have these dark spots. I have tried to cover them with make-up and nothing really hides them. Thank you
