What Do Jitter Feeling With Racy Heart In A Person With Hypertension Suggest?

I m a 53 year o ldl female with CAD, hypertension, diastolic heart failure, hyperlipidemia and pulmonary hypertension status post CABG and 5 stents. Last night I started feeling jittery, heart racing, chest pressure and shortness of breath. Do you think this is a panic attack or cardiac?
