Hello,
Welcome to healthcare magic,
I understand that you have painful bumps around anus. In view of sexual exposure history I am thinking of a possibility of genital warts. It could also be external hemorrhoids. Without an opportunity to closely examine the lesion, I would not be able to accurately specify the condition. You might need to consider a consultation with dermatologist.
Hope this helped,
Take care.
