Female patient 14 years old presented with ascites, odema of the lower limbs and hypertension plus pallor Ultrasound revealed organomegally (liver spleen and kidneys )liver biopsy revealed grsnulomatous liver disease ( epithyloid cells and macrophage but no eosinophil )PCR for tuberculosis in serum and ascetic fluid was negative ,serological tests were negative except for slightly reactive anti Smith and lupus anticoagulant .renal biopsy revealed picture of focal segmental glomerulosclerosis ..ESR was 150mm in the first hour ..he has also mild pleural effusion and pericardial effusion requiring aspiration ...Echo was done and there is systolic dysfunction ..CBC there is low haemoglobin 5.8 gram but wbcs and platelet are normal ..serum creatinine was 1.3 mg/dl reticulocytic count was 6% electrolytes were Norma he has short stature 139 cm and his weight is 39 kgm total serim bilirubin was 1.2 mg ....he recieved 4 bag of packed RBCs in the last 6 months he is highly intelligent but with delayed puberty ..what are the differential diagnosis .. FSGS can occur due to various causes like autoimmune disease, viral infectio etc. The cause of FSGS needs to be investigated and thus treated. Presence of anti smith antibodies and lupus anticoagulant along with granulomatous liver disease and FSGS are suggestive of autoimmune disease. I suggest you to consult nephrologist and hepatologist for management.

Hello, The various investigations shows that the child is suffering from multi-organ pathologies. Liver biopsy shows presence of granulomatous liver disease. Kidney shows focal segmental glomerulosclerosis (FSGS). FSGS can occur due to various causes like autoimmune disease, viral infectio etc. The cause of FSGS needs to be investigated and thus treated. Presence of anti smith antibodies and lupus anticoagulant along with granulomatous liver disease and FSGS are suggestive of autoimmune disease.
