Dear Dr. Robert Galamaga, I need your help in the diagnosis of my daughter in law , her CBC was (2month ago) : WBC 2.7 Leukopenia with slight lymphocytosis lymph% = 51%, platelets 400 , and her HB ( hemoglobin) is 12.5 Her CBC after 2 months , HB is 12.2 , same WBC 2.7, lymph increased to 62% other entries are in range would you please help us to diagnose this case we recommend her to do ANA ( Anti Nuclear Antibodies) and blood film ( waiting for results) , Please let me know your recommendations for this specific case ...Thank you
Basically her blood report indicates bicytopenia (low white cell and platelet counts). To comment on probable cause, I would like to get few more details. Any precipitating factor like febrile illness / drug intake? Any symptoms like fever / weight loss / anorexia? Any presence of enlarged lymphnodes / spleen / liver on examination? How is the ESR?
Above details, along with peripheral smear report will give clue towards diagnosis. This may follow a viral illness like mononucleosis(but we usually expect some recovery by 2 months), or may indicate bone marrow suppression from drugs, or hematological conditions like lukemia and autoimmune condition like lupus (hence ANA was ordered). Your doctor will discuss in detail.
