General Surgeon 's  Response
Hello,

* Hirschsprung's disease is a defect of nerve plexus in which there is abnormal migration of neuroendocrine cells and so there is no development of Myenteric Aurbech`s plexus involving rectum and hence there is large dilated sigmoid colon proximal to the same with distension due to loaded stool.

* Usually it is diagnosed within 2-3 days of birth as there is Squirt sign positive as the infant is unable to pass the stool and doing per rectal examination, there is gush of stool with explosive sound.

*Rectal biopsy is best measure to diagnose the Hirschsprung disease.

* Rest of the issues as malabsorption syndrome from lactose intolerace or congenital lactase deficiency has to be sorted out here through metabolic profile evaluation.

Hope I have answered your query. Let me know if I can assist you further.

Take care

Regards,
Dr. Bhagyesh V. Patel, General Surgeon
