Hi,
Thanks for using healthcaremagic.
The central part of the kidney is called a pelvis.
Extrarenal pelvis is a condition in which the renal pelvis is outside the normal renal parenchyma, so it becomes prominent and looks like hydronephrosis to untrained eyes. It is a normal variant and very common but without any significance.
Severe hydronephrosis will be due to an obtructed renal collecting system which is the reason for the pain. UPJ obstruction is a reason for it.
If it is intermittent, I think you might have a partial UPJ obstruction which becomes significant once in a while. You can visit a urologist, he will analyse your problem and decide whether it can be corrected by medications or if the obstruction is significant, you might require a small surgery to relieve it.
Hope it sorts your problem and wishing you a speedy recovery.
