Hi,

Cold sore presents with a fluid-filled blister and it will be painful. Cold sore is associated with fever.
If it is a solid bump, it could be an inflammatory reaction of lip which could be due to a variety of causes. You could apply an antibiotic ointment if you have raw area on lip. But, I highly recommend you to consult your Dermatologist for pinpointing the diagnosis and for providing proper treatment.

Hope I have answered your query. Let me know if I can assist you further.
Regards,
Dr. Siva Subramanian,
