What Does An Indentation On The Shin Indicate?

My husband has an area about 1 1/2 diameter on the right front side of his shin. It is slightly indented and from time to time, maybe 2-3 times a day, he gets a sharp pain like an electrical shock in that area. He says it s very painful...but it lasts only a few seconds. History: heart disease, apnea, recent spine surgery (scoliosis; L10-T1 spinal fusion) I believe. That was June 1, 2017. This was at Walter Reed. He was there 2 weeks due to acquiring UTI which developed into blood infection, e-coli. From there he went to rehab at nursing care facility. He has made good progress from unable to walk to wheel chair to walker to cane, now. He is overweight at 230 lbs and 5 5 .
