What Does Bleeding During A Bowel Movement Indicate?

I have had severe pain in my left upper quadrant. Felt like my colon went into spasms, then pain traveled over to right upper quadrant then all the way down the right side and to my back. Today when I had bowel movement, the the first time, there was blood in the toilet. The second bowel movement, there was some clots of blood in toilet. My doctor put me on Flagyl and cipro. I haven t felt good for about 2 weeks now. Was diagnosed with IBS 2 years ago. Should I be concerned and ask for any test to be done?
