What Does Chest Pain While Suffering From Hypercholesterolemia Indicate?

A 36year old man with a history of moderate hypertension and hypercholesterolaemia with ocassional complaints of vague left sided chest pain and as well as a gripping pain in the chest. Poorly compliant with amlodipine and atorvastatin. Acute complaints of left back pain and shortness of breath and was placed on aspirin. Suddenly developed reduced sensation in left hand and leg. No slurred speech or facial palsy. Bp in the last two weeks within normal limits. E/u/cr normal. ECG normal. Yet to do cardiac profile. I m thinking of a probable transient ischaemic attack with background myocardial infarction. What do you think and how should he proceed ?

We will try to help you in best way possible. First of all it's sad to know what your patient had been going through lately. Based on the history you have shared, you are quite right as it needs to be worked up for both myocardial infarction and cerebral infarction as patient is highly at risk to catch both, and patient should ideal have a MarI brain with diffusion weighted images as well as ECG and Echocardiogram and if needed ETT and angiography may be considered. Meanwhile anti-platelets and statins should be continued.
