What Does Internal Bleeding With Low Vitals After Knee Replacement Mean?

my relative is in the hospital recovering from a knee replacement he had on Monday. Today, his vitals went down (BP 87/34), is bleeding internally and the hospital can't figure out whats goin on. We have asked that he be transferred to a highly regarded hospital 60 miles away and were told we would have to wait a day or two. were afraid he is goin to die, he has gotten much worse in the last 4 hours. What steps do we need to take to get him transferred immediately?
