Pulmonologist 's  Response
Ghanks for your question on HCM.
There are many causes for right upper lobe lesion.
1. TB
2. Pneumonia
3. Bronchiectasis etc.
TB is very common in smokers due to poor lung defence. So your husband may have TB.
Better to consult pulmonologist and get done sputum examination and bronchoscopy and BAL analysis for TB.
If he founds positive than he should start Anti TB drugs for 6 months.
First 2 months H (isoniazide), R (rifampicin),Z (pyrazinamide), E (ethambutol) and next 4 months HR only.
And yes during 1st moth he is infectious.
