What Does It Mean To Have No Gestational Sac In Trans And Abdominal Ultrasound?

Hi my wife had u/ s at the er at 5weeks and hcg level was 4100 and they did both u/s trans and abdom nothing showed up no ges sac we had ivf and transferred 2 emb. Two days later hcg was 8993 we have a mother u/s at fertility doc in 10 days what does this mean
