Welcome to health care magic.

1.The cortico-medullary differentiation is lost in case of medical renal disease ( in presence of any kidney disease )
2.This can be correlate with the lab reports like urea, creatinine,Albumin status.
3.And left kidney shows lobulated contour - its seen in dormitory kidney - its one of the normal variant of the kidney ( rare).
4.Other possible cause is presence of any mass lesion in the kidney - can be confirmed by a computer tomography with contrast ( that will help to evaluate with enhancement )
5.I would suggest to get KFT - kidney function tests done ( blood test) then further as the results show. Good luck.

Hope i have answered your query.
