Looking into your narrative it is clear that you are suffering from prolapsed inter-vertebral disc. As you have not mentioned your age in the narrative it is very difficult to plan the treatment because age is very important determinant as far as back pain or disc prolapse is concerned. But from your history I think you are most probably in mid 30s or 40s so you just have to take some pain killer like tab ultracet twice daily or tab biozobid-plus thrice daily after meals along with tab shalcal 500 mg or tab toscal-gem once daily after meals with tab rantac or cap omez twice daily empty stomach. You can also use volini gel or powergesic-plus gel for local application. It is also quite beneficial in relieving the back pain and gives you some relief. Apart from these you can also go for some physiotherapy exercises after an expert opinion of physiotherapist. Also you have to sleep on a plain surface because it is a good practice in case of back pain which is due to prolapsed disc.
