Hello. Presently your Total PSA is mildly elevated (must be less than 4) wherear your Free PSA is within the normal range (must be less than 1.5 by most lab standards). More important than absolute value of PSa, it is important to judge how fast and how much it is increasing. Depending on age, most elderly males will have prostate symptoms and elevation of PSA. Tests such as PSA velocity are more important. If you are symptomless, you can get a repeat test after 1 month. If it increases, it is a must to consult a urosurgeon. If you are presently symptomatic, you must consult at the earliesta TRUS will help better to correlate with your PSA levels. If needed a biopsy can be taken at the same time. Take care
