What Does My Blood Test Report Indicate?

My CBC shows that I have a low platelet count (fluctuating between 50K and 75K) as well as high HCT (between 48% and 57% over the last year) as well as Hgb at around 18.5-21.5 range and have had an ultra sound revealing that I have mild splenomegaly. My bone marrow biopsy was normal, so polycythemia was ruled out for now. My hematologist is stumped and has been treating me with phlebotomy every few weeks to keep the HCT down. Has anyone out there have any ideas as to what could be causing this or a possible diagnosis?
