Hi,
Thanks for writing in.

The pouch of Douglas is an anatomical area behind the uterus which usually does not contain any fluid in normal circumstances. In many women little fluid can be seen during the time of ovulation (14 days before onset of periods) and this can be considered normal. At any other time of the cycle, fluid in this region can signify pelvic inflammatory disease.
