my father (age 59) has suffered from two mini stroke in 2004 and 2009,now in 2011 we had a ct scan the report stated: multiple small low density areas are seen in left putamen, caudate nucleus, left corona radiata b/ thalami.
generalized widening of cortical sulci is seen bilaterally.both sylvian fissures are also widened.
both lateral and third ventricals are dilated.periventricular leukoariosis is seen.
there is no mid line shift.
brainstem & cerebellum appears normal in morphology and attenuation.
impressions: age related changes with periventricular ischemic leukoencephalopathy & lacunar infracts in left putamen, caudate nucleus, left corona radiata and b/l thalami.

Hello
Thanks for writing to HCM

Age related changes with periventricular ischemic leukoencephalopathy & lacunar infracts in left putamen, caudate nucleus, left corona radiata and b/l thalami are mainly due to chronic microvascular ischemic changes resulting from involvement of small blood vessels.
Iischemic changes resulting from blood vessels changes in diseases like hypertension, diabetes mellitus, dyslipidemia.
So,your father need monitoring of conditions that leads to ischemic changes like hypertension, altered lipid profile, diabetes mellitus.
He need investigations like routine hemogram,RBS,LFT,RFT,Lipid profile, ultrasound of abdomen.
Cerebral cortical atrophy is age related finding and doesn't require any treatment.
He is getting right treatment and he should continue it.
He should totally avoid alcohol and cigarettes.

Hope i have answered your query.
