The following are few causes of severe pain in joints:
1) Osteoarthritis
2) Rheumatoid arthritis
3) Gout
4) Septic arthritis
5) Arthritis associated with infections elsewhere in the body like urine infections.
Usually toenails
fungal infection can't cause pain in muscles and joints.
The following is treatment suggested:
1) NSAIDs like tab. Ibuprofen or tab. Naproxen are to be used for 1 week.
2) As the above medication can cause gastric irritation, tab.
Pantoprazole 40 mg is to be taken once daily before food.
3) In patients with bleeding disorders and kidney failure, NSAIDs are not recommended.
4)In such patients, it's better to use opioids like Hydrocodone, Oxycodone or Tramadol daily twice for 1 week.

Physical therapy is advised to strengthen the muscles and joints.
If pain doesn't come down even after 1 week, you may need to
consult a rheumatologist for further evaluation. Regarding toe
nail infection, topical application of nail on helps eliminate the fungal infection.
