What Does Severe Pain In The Anus Indicate?

My wife has awakened with severe pain in her anal region. She stood up and lost consciousness, falling in the bathroom and vomiting. This has happened before (2x s)- last time we called the fire department guys and then went to emergency. Tests at emergency did not pinpoint anything wrong. Neither did the colonoscopy (sp) . My wife is a 53 year old PE Teacher in good physical condition. Thank you for your availability!

As all the tests are negative, please note that anal pain with loss of consciousness points towards psychic reason. So, please take the help of psychiatrist.
