What Does Skull Depression On Head While Having Fatty Liver Indicate?

My son, 17 years old, have fatty liver and is 300 lbs.,have recently found 2 skull depression on the top side of his head. It's about 2 inches long. He had never had any head injury, and didn't feel any pain. What could it be? Is it serious? Thanks.
