Generally kidneys become small due to chronic renal disease resulting form chronic hypertension,diabetes mellitus etc. Since cortico-medullary differentiation is well maintained,I don't think kidney size is a matter of concern. Difference of 1.5 cm in 10 days may be due to technical error.It is not expected to decrease without any systemic illness. She may need complete renal renal function test(RFT).If RFT is fine then it is not a matter of concern.
