What Does Soreness On The Tip Of The Penis Indicate?

Hello, I am recently having some issues. I have a uncircumcised penis, and lately the tip of the for skin has been kind of sore when I pull it back too clean it I notice there is a few years (cut) like marks on it and it hurts when I pull it back. I was wondering if I should be alarmed or is this something that will go away. I am sexually active and it has happened before but not as bad. It s now to the point if I urinate and it gets touched it stings. I have heard of applying honey too it as a natural ointment would this work?
