What Does Stiffness In The Jaws And Pressure In The Ears Indicate?

Hello there, I am a 21 year old female and for about a month and a half now I have been experiencing muscle tension in the TMJ area. It all started when randomley i woke up about a month and a half ago and noticed that my neck was unusally tight on the left side (no trauma). Naturally, I kept trying to massage the muscle to ease the pain and eventually after two weeks it seemed to have dissappeared (stayed like this for an additional week). After the week of no pain, i noticed the neck stiffness had come back. This same week i had a cleaning done on my teeth with no change in the stiffness of my neck. The following week (week 4) I had 4 fillings done on my teeth, and within a coupple days after this i noticed that this stiffness had spread to my jaw and ear area. My masseter muscles on my jaw began to swell and I also had an ear infection all on the SAME SIDE. I was treated for my ear infection with a corticosteroid which cleared the infection but i still feel some sort of pressure on the left ear. Anyways, I went to several doctors including my family doctor regarding the issue and everyone has told me it s all muscular and that i should try massage therapy but at the same time were 100% about their diagnosis. So i booked an appointment with a chiro and had an adjustment done on my neck and noticed pressure released from my neck but still seemed to have the inflammation and swelling/ pressure in my jaw ear area. A week after this the neck pain has come back, and i noticed a small ulcer like sore on the top side of my left gum near cheek area. Since i was about to go crazy from all of this 2 days ago i decided to contact my dentist again and he looked inside my mouth and examined my jaw and confirmed it s not an infection and prescribed me to take robaxacet for my muscle inflammation around my jaw (masseter), baking soda and water for the sore and also told me to wear my mouth guard every night to decrease grinding. That same day i decided to go see my massage therapist and he proceeded to examine the TMJ area and did some work on it. He claimed that my entire left side of my TMJ area was lit up or being stressed. Following all of the medication that i was told to take and the massage, that night i noticed an INSTANT decrease in the inflammation in my jaw area and noticed a little bit of release but still pressure in ear area. I had no problem falling asleep but It is now the next day and i noticed that my jaw neck pain had come back and through the day it seems to come and go. I have an ultrasound thursday booked to see if there is any lumps or bumps causing this pressure. I am a smoker.. and I have actually had to stop (not a bad thing) because I feel like when i smoke that area becomes tense and inflammed for some reason. I also notice that when i get anxious or scared that this area becomes tighter. I am at my wits end with these ongoing symptoms on this left side and I need answers!! Any thoughts or reccomendations would be greatly appreciated. Thank you!

Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advice.

Well, I have read your complete explanation and the stiffness in jaw, ear pressure and neck tightness is all appearing to me related to muscles but the point of origin seems to be jaw joint.
As you mentioned that the pain and stiffness increases when you are stressed and also the dentist mentioned teeth grinding, therefore teeth grinding or bruxism and teeth clenching can be a cause.
So my suggestion is to get an x ray of the jaw joints that is a Panoromic x ray done so that the exact cause can be ruled out..
Also you can get the ultrasound done for further diagnosis.
However it looks related to the jaw joints only.
So for now you can take muscle relaxants like Chlorzoxasone and also do alternate warm and cool compresses.
Do meditation and deep breathing exercises so that the stress is minimized and the bruxism and clenching reduces.
Take soft meals and avoid excess mouth opening.
Continue massaging the muscles in round gentle movements of finger tips..

Hope I have answered your query. Let me know if I can assist you further.
Regards,
Dr. Honey Arora
