What Does Swelling Up Of Feet During Pregnancy Indicate?

My daughter lives in Guam USA. She is very active and always hiking,swimming, even running marathons. She has had some injuries over the years but nothing major. She found out she was pregnant in January, and around 6 weeks of pregnancy she started having swelling in her left lower extremity. The swelling gets better but never goes away. There is no rhyme or reason to the timing of the swelling-she can go to bed with very minimal swelling and wake up to a huge lower extremity. Being up and on her feet doesn t make it worse, and sometimes gets a little better. Her OB doc says it s just due to pregnancy because of the increased pressure on the vessels as the uterus gets larger. Maybe so, but it started before her uterus had just started enlarging. Someone mentioned elephantitis to her, which could make sense--tropical climate, lots of mosquitoes, and an island formed by volcanic activity. If it s some kind of parasite causing it, should it be treated asap, and can it be treated in pregnancy? Just grasping at straws here. Maybe I m way off base but a couple of docs I work with brought up elephantitis but they don t know much about it or the treatment.

Hello,

Suspicious of lymphatic filariasis until proved otherwise. I have gone through the history diligently and I am of the opinion that based on unilateral swelling (left lower extremity), onset before pregnancy and the environmental factors are suspicious of lymphatic filariasis.

She needs to be evaluated clinically by a physician and the diagnosis may be confirmed by the identification of microfilariae in a blood smear by microscopic examination the sample of which should be collected during night.

Diethylcarbamazine (DEC) is the drug of choice, unfortunately, it is contraindicated during pregnancy.
