What Does The Following CAT Scan Report Suggest?

HI my name is Jerri . I have quite a few things going on. I was diagnosed with hemochromatosis and polycythemia vira about 10 yrs ago. Next I got portal vein thrombosis. In 2014 I was diagnosed with Lyme disease. And most recent diagnosed with CREST disease and unceritive colitis. I went in for a cat scan last Friday and just now picked up results. It says : Progressive, now severe enlargement of the spleen with mass effect of surrounding structures. Cavernous transformation of the right portal vein. Elevated right hemidiaphragm.
