What Does The Following CT Scan Report Suggest?

in a case post operative nephrectomy renal cell carcinoma clear cell type one year back developed a cough since 4 months.On revation the pt ct showed two irregular nodular hypermetabolic lesions larger one mearing 1.8x1.6x1.6 cmsand showing central cavitation seen with in the anterior segment of left upper lobe.Tiny bilateral soft tissue pulmonary nodules and enlarged right hilar lymphnodesare seen.Fnac done shows scanty cellularity consisting of of few clumps and groups of cells with pleomorphic nucleus and granularr cytoplasm.in the background of plenty of inflammatory cells,lymphocytes and histiocytes. Meatatic deposits of renal cell carcinoma in lung.In this can direct chemotherapy(votrient) or surgery can be done without confirminig with biopsy?

Yes, treatment if any is needed can be started on these reports. In fact the clear historical and CT finding with a clear picture on FNAC; treatment may be started and biopsy is not needed as diagnosis is clear and not in any doubt.
