Well defined nodule measuring 1.2x1.2 cm with immediate subcentimeter nodule medially is seen in the left upper lobe. Surrounding fibrotic residual is seen anteriorly. A lobulated mass measuring 3.4x2.68 cm is seen in the posterior segment of the right upper lobe. Surrounding ground glass opacity noted. Hilar ground glass opacity is noted. Subpleural nodule measuring 2.7x2.4 cm is seen in the right upper lobe. Small opacity with fairly defined margin is seen in the right lower lobe. There is no evident pleural effusion. Enlarged lymph node is seen in the right paratracheal,precarinal and right hilar. Heart is normal in size aneurysmal dilatation with a widest diameter of about 5.0 cm is seen in the aortic arch. Incidental note of nephrolithiasis seen in the right kidney. Suggest contrast chest ct scan for further evaluation to rule out oncologic process. Nodules in the report of lung may be due to old infectious disease or cancerous lesions.But since the hilar lymph nodes are enlarged,it suggest its an active lesion,mostly cancerous or non infectious interstitial lung disease. Ground glass opacity is a non-specific sign which may be because of infection, chronic interstitial disease or pulmonary oedema. In this case,there can be two possibilities.Ground glass opacity due to interstitial lung disease accompained with nodule. Second possibilty is the pulmonary oedema caused due to aortic dilatation (dilatation of largest blood vessel supplying heart) Nephrolithiasis means stone in kidney. Above findings needs to be further evaluated by contrast CT scan,tissue biopsy and clinical correlation. Further management depends on the cause.
