What Does The Following ECG Report Indicate?

ECG showed anteroseptal wall myocardial infraction of some duration. Echocardiography revealed RWMA+, mild concentric LVH, severe LV systolic dysfunction with grade II diastolic dysfunction, RV systolic dysfunction with LVEF 30%. Coronary angiogram done on 03/04/2017 showed left main with triple vessel coronary artery disease.
