Welcome to health care magic.

1.The report suggest there is a degenerative disc disease at mentioned levels.
2.The degenerative disc disease seen as disc bulge which is seen causing pressure effect on the thecal sac anteriorly.
3.There is the foramina narrowing noted in the mentioned levels - the place where the nerves pass through.
4.The disc when there is hydration lost - there is a disc bulge cause and causes pressure effect on the adjacent structures.
5.The osteophytes are seen as a degenerative spine which is seen in the age related changes / excessive usage.

Good luck.
