What Does The Following MRI Report Suggest?

My MRI shows small amount of fluid within the subacromial and sub deltoid spaces. Marrow space is intact. No joint effusion. This is post rotator tear surgery. What can I do about the severe chronic pain. I try ice pads and have been on opiads for a long time .
