I can understand your concern. 
I have gone through the blood report you have mentioned. 
This report is classical of 
iron deficiency anaemia (IDA).  
Low 
hemoglobin, low MCV, MCH, MCHC, presence of microcytic, hypochromic, target cells, severe anisopoikilocytosis etc are suggestive of IDA. 
So you should take 
iron supplements (tablets), 
multivitamin (
folic acid, vitamin b 12 and pyridoxine) tablets for at least 3-4 months.
Take iron rich diet like spinach, carrot, Apple, dates, jaggery, sugarcane juice etc. 
Don't worry, you will be alright with all these.
