Hi,I am Dr. Shanthi.E (General & Family Physician). I will be looking into your question and guiding you through the process. Please write your question below.
What Does This MRI Of Spine For Intense Back Pain Indicate?
I am a 36 year old female. 5 7 170. I have been in extreme pain for about 6 months. It hurts worst at night when I try to sleep and in the morning when I get up. I have already tried pain management and epidural steroid injections. Nothing has helped. MRI report says the following: Multilevel lumbar spondylosis with no acute fracture or aggressive lesions. At L5-S1 there is 3.5 mm broad posterior disc bulge and posterior central annular fissure, with mild displacement but no compression of the S1 nerve roots. Discogenic Modic type 1 changes of the endplates identified at L5-S1. Small disc bulges at multiple levels with no central or foraminal stenosis. When I read that, it doesn t sound very serious, but it hurts so much! Should I be considering surgery?
As per the narration of report, the incidence is not that of requiring immediate or emergency spine surgery (which has its own ill effects). There is no mention regarding the physiotherapy and postural adaptations obtained after MRI along with the therapy. If due consideration over the assisted physiotherapy is being given over a period of 3-4 months and the pain is persistent, then surgery may be coming into the picture. If physiotherapy, wax bath, ultrasoundshock waves are not being given, kindly do discuss the scopes of the same in your condition for better results.
Hope I have answered your query. Let me know if I can assist you further.
Regards,
Dr. Bhagyesh V. Patel, General Surgeon
