What Does This MRI Report Indicate?

Hi Dr Herrick, I am 42 and a very keen cyclist. average around 150 miles per week. On Friday I was advised that my MRI had shown up that I have bulging disc in L4 & L5, I always thought that it was my hip causing my pain whilst standing for log periods and laying down but it would seem not. Standing for over 1hr is terrible and any longer it makes me feel sick with pain, so I am always looking to sit down, also I am woken up with it during the night. I do not feel any discomfort whilst on my bike, and my core strength is excellent. Can I still cycle, and over the last few years I have on occasions struggled to hold my wee. Kind regards Stacey

Hi dear , I have gone through your question and i understand your concern.As per history provided you have
Lumbar Herniated Disc.

General symptoms typically include one or a combination of the following:

Leg pain (sciatica), which may occur with or without lower back pain. Typically the leg pain is worse than the lower back pain.
Numbness, weakness and/or tingling in the leg
Lower back pain and/or pain in the buttock
Loss of bladder or bowel control (rare), which may be an indication of a serious medical condition called cauda equina syndrome.

In most cases you feel better in about 4-6wks and following adds acceleration to recovery-

Physiotherapy ***mostly beneficial.
Osteopathic/chiropractic manipulation (manual manipulation)
Heat and/or ice therapy
Non-steroidal anti-inflammatory drugs (NSAIDs)
Oral steroids (e.g. prednisone or methyprednisolone)
An epidural (cortisone) injection.
If symptoms continue after six weeks, and if the pain is severe, it is reasonable to consider microdiscectomy surgery as an option.

Advice:-
Usually 2 days strict bed rest is sufficient with above addons.
I find good to know thay you are cyclist and your age is 40+ but certainly I would recommend to rest for 2-3days and then do exercise which I would include later and gradually start with cycling once improve on symptoms.Cycling in straight & recumbent back is recommended as it aids safety and benefit to Disc herniation.
Exercises:-
1.best exercise to put herniated disc on its place
#lie face down Or prone on the bed with elbows on the side.
#prone prop McKenzie or McKenzie pushups
#Extension of back while standing.
2.Disc stabilization exercises
#Cat-cow position - on your all fours-knees & hands
#Curl-ups and Squats (with chair behind)
